General Liability Waiver 

As the parent and/or legal guardian of the Player named below, I give my son or daughter permission to participate in the Glen Arm Sports Winter Basketball League and I certify that he/she is in good health. Also, I agree that neither my child nor I will make a claim against, sue, attach the property of or prosecute Glen Arm Sports, Open Bible, Highlands or Coppermine for injuries or death which my child may sustain as a result of my child's participation in the Glen Arm Sports Winter Basketball League.

I fully understand that participation in the Glen Arm Sports Fall Basketball League may involve serious risks and danger that may result in harm, bodily injury or even death. I also understand that basketball may involve physical contact between players, that serious accidents occasionally occur, and that participants occasionally sustain serious injuries (including death) and/or property damage. Knowing these risks, I agree that my child and I assume those risks and release and hold harmless Glen Arm Sports, Open Bible, Highlands, and Coppermine who might otherwise be liable to me or my child for damages.

Also, In the event of an emergency or an injury occurs, I authorize the Glen Arm Sports Staff, to take all proper action and use the emergency service available at the nearest hospital if necessary. I understand my personal insurance will be used in this case. I, for myself and my child release and hold harmless against any claims, damages, and expenses Glen Arm Sports (including staff), Open Bible, Highlands and Coppermine  & and if applicable, owners and lessors of the premises used to conduct the event.

I acknowledge and agree that Glen Arm Sports retains the right to use photographs and videos taken of the event participants for publicity and advertising purposes.

BY COMPLETING THIS FORM, I ACKNOWLEDGE THAT I HAVE READ, UNDERSTAND AND AGREE TO THIS LIABILITY WAIVER AND AM SIGNING ON BEHALF OF MYSELF, MY CHILDREN AND/OR THOSE MINOR CHILDREN THAT I AM A LEGAL GUARDIAN, FOSTER PARENT OR CHAPERONE FOR.
